Help with Microsoft Dynamics CRM

We have designed a simple contact form for a specific client engagement. We would like to build an additional section on the form to track specific data from the client. This data needs to be collected in a way that we could aggregate across the client engagement and at different points in time. For example, there are 3000 clients. We want to collect data on 10 financial wellness concepts -- do they have a will, yes or no? Do they have guardians assigned - yes or no? We then want to be able to pull the number of wills across the 3000 clients. Additionally, in the social pane, we would like the ability to set an appointment (from the contact form) and have it directly be added to the calendar of a user.
