Create Wordpress Plugin

I have a lot of experience in developing plugins for WordPress and believe I understand the requirements of your project completely. I would propose developing an admin page where you can select/create which form fields you would like included in the widget. Then be able to place the widget as usual on the widget page. On the front-end, I would propose on submission of the form making an AJAX request in the background with a small jQuery popup informing the user of their request - whether fields are missing, whether submission of the form was successful or not, etc...
