I have attached a file that shows a very rough mock-up of the "Service and Repair Request" Form our business would like implemented. Upon creation, this will need to be copied across to our 4 websites. We have attempted to reduce its complexity and simply create useful data at 3 points in the process, which I have colour coded on the spreadsheet (details of which are included in the "Extra Info for Programmer" section, but please don't hesitate to ask if I've made no sense).

Additionally at the end of the process, the client will come across a detailed content page, with the majority of text having already been included for the designers' ease. We want basically ALL of our questions to be compulsory (which I have indicated by the word "compulsory" and red asterisks), and for submission to be unavailable to under-completed forms.

There are a number of dropdown box options required, as well as some minor content. Additionally, we require for our company to have the ability and access to edit this form ourselves in future (i.e. mainly the content on content pages).

To even be considered for this job, you should have read this project description. You need at least 3 years experience with Neto software, and the first line of your Project Proposal should read: "NETO EXPERT: Hi Chloe".
