Convert a Template to a Website

Its a straight forward job, I have a template which needs to be converted to html page. The webpages will be made using bootstrap. The png file for it has been attached and I will provide .ai file to the selected designer.
Two pages are needed to be made. One page will be made exactly like that shown in the attached file and an another page will have blank space (for message) instead of the form(check image).
Please note that I need the work done by tomorrow noon (IST).
