CSS & HTML Requirement for WordPress Website

I have a small html and css requirement. I need someone to write the css code (and changes to the html code) that will style the below whmcs search module integration code into my WordPress 3.5.1 website. Here is the integration code I need styled www. .com .net .org I already have the images i want used and will supply more information when the project is awarded. (images & examples etc.) I need 2 versions of the search box. 1 with the prices included (930px width X 55px height) and 1 with the prices excluded (700px width X 55px height) Requirements: 1. The code for my wordpress site and where to put it. 2. The css for my stylesheet 3. I will want two version of it, 1) 900 x 66 and another 2) 650 x 66 4. It must work in WordPress Many thanks,
