Convert 12 page PDF document to 1 page HTML Form

I need a 12 page PDF converted exactly to a single page HTML file. The HTML version you create must print exactly as the PDF does across 12 pages from within IE 9 and Safari. It needs to be HTML and CSS only with images used only for logos. The areas of the form where data is entered need to be blank so I can insert PHP generated text. The HTML version you create needs to be a blank template that I can use to automatically fill out with PHP. I need the Master PDF file converted to HTML. I have included cleaner Original PDFs that you can use as a reference that form part of the Master PDF. The dark areas of the PDF where titles are should be a blue #06C.
