Hi I need a secondary registration form that will be used for customers to register on location using a tablet. It is NOT a replacement for the existing form. I have included a psd file that should be used as the template for the form. The form should also be linked to my mailchimp mailing list. All fields displayed are required and an ajax notification should be shown for any errors. Will need you to create any additional fields on Joomla and Mailchimp.
