Custom checkout questions for shopify

We need to collect three bits of custom data from customers in our Shopify checkout process.
1. Patient Name: (first and last)
2. Patient Date of Birth: (Month/day/Year)
3. Patient Gender: (M/F)
These need to be required fields for each order. I also need this data to be included in the confirmation email.
