create 3 fillable pdf forms - for Kotenko

We need three fillable pdf application forms created. These are for three different versions of a summer camp. One for photography, one for climbing, and one for adventure. The three forms will be almost identical, only the header will change and a the climbing & adventure versions will need extra boxes to select a session (2 session choices for adventure, 3 session choices for climbing). They will be based on our existing print brochures (attached). They are for printing, so need to fit on a 8.5 x 11 inch sheet of paper. They will need to be formatted so that users can save the form (with data) after they have filled it out.
