convert 3 forms to save to mysql and export to Access

I have 3 forms in html, they have 15 to 50 fields each one, and i need to have the forms save the results to a mysql database. The forms have some required fields which i'd like to have server side validation for. Additionally, I'll need a script to export the records from mysql to MS Access via an 'export' button. I'll need a 2nd button to 'delete' all records from the database as well.
