Debug PHP script - minor graphics & other fixes

PHP script has two minor bugs: 1) Fails to increment 4 variables based on user's response to questions. 2) Fails to display pie chart (which is supposed to show the percentage of user's answers in three different categories). The pie chart script works fine on its own, but produces errors ("function not found") when embedded in the main script. I will provide the main script ([login to view URL]) and the pie chart script. The [login to view URL] program is commented, showing where the problems seem to be. I need the [login to view URL] program fixed asap.
