Code Dynamic Contact Form

We've generated a simple interface for a MySQL db using PHPRunner (familiarity with this tool is a plus; uses Smarty). The app is shows a list of chapters with a link to a display page with complete contact information for each chapter/record. Our client does not want e-mail addresses revealed on the display page.

We designed a contact form as an external link but want to pull data from the display page (along with some additional fields from the database) to e-mail to the chapter. We tried using the GET/POST method, but do not want to pass all the info through a URL.

We need someone to help with this functionality. Specifically, we need:

1] Coding to carry a record ID field and 4 other fields from our "view" (display) page to the contact form

Note: Several of the fields are NOT actually displayed on the page (they are stored in the db, e.g. email1, email2, email3)

2] Pull those db fields into the contact form for processing. The actual form consists of the user's name, email, a subject and message.

3] Optionally, there can be an external form processor or something like formmail (with hidden inputs for e-mail variables) that will then send e-mail to db email addresses and redirect to a thank you page.

Our expertise is in creative services, so we'd like a PHP guru who understands sessions/variables to code this piece.
