I am working for a speech pathologist that has to fill out several (~7) forms detailing the patients she treated on a monthly basis. The work is very repetitive and has been done by hand up till now. This project requires a programmer that can create a database in which the speech pathologist can store patient information, and also a way to export specific columns of the database into separate Excel/Word tables. No particular table will use ALL of the information in the database - rather, the tables have to be composed of specific columns from the database depending on which form is being filled out. The idea is that the speech pathologist will only have to enter the information into a simple form every day, and the program will "spit out" the tables and information that is required at the end of the month.
This project has already been started, but the programmer will be able to start from scratch if he/she finds a more efficient way to do this. It need not necessarily use SQL or PHP as long as it can do what is needed.
As of now, there exists an HTML form that stores the required information into an offline SQL table using MAMP.
