CLEAN UP CSS and LAYOUT OF MY ORDER FORM!

Hi there. I have a order form template. Simple blue table with grey background.. White Background inside table.. Two Columns.. Upsell Box.. Typical Order Form.. HOWEVER - I cannot get it to look right because of the two columns and CSS .. SO I NEED SOMEONE TO CLEAN IT UP! 1) I will upload the template that I have right now. 2) I will give you TWO different order forms that I would like you to draw from. 3) Then, I would like you to make my order form look like theirs. 4) If you can't use my template, then please make one that I can use. I'm looking for this right now. Should take 1 hour to 2 hours at most. If you cannot finish this right now - then please do not bid. My programmer is waiting for this file. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
