Add AWeber Email Form HTML

I have the graphics for a simple form set up on an HTML site and already have my email input form created in my AWeber account. I need someone to take the raw HTML that AWeber has provided and "connect it" to my form on my site. In other words, I want to use MY form and not AWeber's to collect an email address and send it to my AWeber account.

I'll pay someone $10 to take the raw HTML from AWeber and connect it to my form. You can visit the site at [url removed, login to view] to see the form. All you have to do is email me the HTML. I will upload it.
