Custom Word Press Pop Up Plugin

Would like a customized popup plugin to use for multiple wordpress themed websites. The basics must include: Easily include imagery and customize the look and feel of the popup for branding Customized buttons Ability for multiple unique popups per website (depending on the page they are visiting) Capture form field without using a email service provider like Constant Contact Easily set how often the popup appears (such as 1x per day, 1x per visit, etc) Ability to set when the popup shows (seconds, when exiting the page, exiting the site, etc.) Buttons that once they submit or select a hyperlink within the popup the user is redirected to a specified landing page There may be one that already exists that can be implemented or one that can be customized.
