Create bbPress Theme/Template from HTML page

We have an HTML page that we need converted into a bbPress theme / template ([login to view URL]). You need to take the HTML code and convert it to a completely working theme - you will need to test and confirm that all public pages function and display the theme using bbPress 1.1 ([login to view URL]) - this is for the standalone version, not the Wordpress plugin version. Please note this is not just for the main forum page, you will need to modify the theme as required to work with all publicly accessible pages including profile, topic view, and others. See attachment for the style of the forum home page (we can supply both HTML/CSS & sliced-PSD as required).
