Code the most basic Wordpress theme from my designs

We need you to create a very basic Wordpress theme. We have designed a page template which we should be able to choose when creating a page in the admin system (see link to page design below). We need this done with page templates please, and not just coded in the [login to view URL] file because we will be adding further templates at a later stage and need to be able to choose between them.
The menu in the header should be dynamic, using the built in WP menu system, and the logo and phone number will be static.

This is simply a header and some content. The headline/image/content will all be put into the WP page editor, so you don't need to code these as separate elements.
