Aweber Email Optin Form For WordPress Pages

This is a simple job for a PHP programmer that knows how to code. PHP Programming for a form code that will allow me to place my aweber email optin list codes in the form and have the simple form show up anywhere on a WordPress post or page that I want the form to show up, whether it be between text or in the middle of the page, I need to have the ability to have this form show up where I want it with simple adjustments. This form will work independantly for separate Aweber email optin lists that I offer on separate pages on the same wordpress domain name. For example, I want to add this email optin form on one blog page and have one email list campaign input box show up on that page to build an email list for one campaign and then I will want to also use the same form for a different page, but have that new form be a totally different Aweber email campaign, if that makes sense to you. Basically, i want to load one form on one page and then be able to load the same form on another page and have both forms work with a different Aweber list. I think there will need to be some place that allows me to input my Aweber list form codes in each of the forms for the particular Aweber email list building campaign. I will supply you with the simple email form image and the button. You only need to build the form around my image and button.
