Add fill in fields to existing PDF form, make merge-ready Word document

My business needs to complete 500-1000 of a couple of New York State forms. The forms can be downloaded as PDFs from: <[login to view URL]> (Form RR2A) <[login to view URL]> (Form RR2S) The problem is that these forms are not fill-in PDFs, so we need to insert them in a typewriter to complete them. As all my information is in a database anyway, what I would prefer is to have each form as a Word file, with the PDF inserted as a background graphic, and transparent Word text boxes inserted in appropriate places throughout the forms. Then, I could place appropriate mail merge fields from my database into the text boxes and do the forms as a mail merge, instead of having to type each one individually. So, I’m seeking someone to create these Word documents with text boxes for me. ## Deliverables Two Word files, one for each PDF, with the PDF as a background image that does not move is visible through the text types in front of it. In each file, an appropriate sized text box needs to be inserted in front of each blank and checkbox on the form, so that I can insert mail merge fields into the appropriate places. A more fully detailed list of the fields I need in each file is attached as a Word file and is part of this bid request. It’s important that the image quality / sharpness of the PDF not be reduced when it is inserted as a background image into the word document. If you need to convert the PDF into some type of graphics file first and then insert that file as a locked background image into the Word document, that’s fine, but you must use a method where the graphics file is basically as sharp and clear as the original PDF. Note: Form RR2A has three pages, I only want the Word file to contain the first page of the PDF, the detailed form. You don’t need to do anything with the last two pages of the PDF, they should not be part of the Word file. While we will fill out most forms by mail merge, we may need to enter data in some forms manually. I would prefer a solution where the documents are set up as Word fill-in forms, with a tab order in order of the items, but where I could still enter prefilled data or define merge fields into the fill-in boxes instead.
