CV / Resume PHP Script

I am looking for a PHP script not a whole website. The purpose of the Script is to collect the users information and present it in a CV format, one of a choice of formats ideally. This data should be able to be stored in a mysql table for retrieval and the selected CV format saved out as PDF. A Spell checker would be nice.
