Create PHP & jQuery Proxy Script

I need a script that is like [login to view URL] It must have a bar at the top, with a URL input field, where a user can enter a website address, along with buttons next to it for popular sites. I would like to to be based on Twitter Bootstrap v3, and the buttons for popular sites should be text only. I have created a HTML mockup of what I would like it to look like, the mockup is attached. It must use AJAX to load the website into the iFrame below the bar, and should have a loading bar like the one on YouTube. When a user visits it, they should be prompted to login using a Modal window, so that they can see the website behind, and see that it works, but cannot use it till they login. There does not need to be a registration modal as I will add users directly to the database. Once they have logged in the modal should disappear allowing them to access the content behind. When they enter text into the search box, it should check if they have entered a URL, or a search string, and if they have entered a search string it should load the [login to view URL] results for that search (like the search/URL bar in Chrome). The iFrame should load content using some form of PHP proxy, so that it works even if the site being loaded is blocked by the country it is in. It is up to you how the PHP proxy system works, but it MUST work to load things like YouTube or Facebook from within China, just like Securitales.
