Convert a webpage template from PSD to HTML

Looking to convert a newly edited PSD website template into HTML in the next day or two. I have basic HTML skills and will need to edit some of the text later on, so your coding must be consistent with HTML programing standards. There are 4 pages in total to be converted, 3 of them will have almost no content on them. Ill need you to link a data input box (for peoples emails, phone number, etc) to send any submitted info to my email address. I'll also need you to upload it to my hosting and link it to my domain name so that it is fully functioning. I will need it within 2 days of choosing someone for the job.
