Create simple application for data collection

We are looking for an experienced programmer/developer to design a basic custom application for our business. Currently, employees fill in a one-page paper form when they visit our customers and then submit the forms to our office. The data is then input into a spreadsheet and then we perform basic analysis on the data. We are interested in creating a simple application that will make this entire process electronic. One part of the application will need to reside on the employees tablet PC (or notebook PC) and will be used to collect data through an electronic form. The other part of the application will need to be either on a PC at our office or on our shared-hosting server. The application will need to include the following: 1. An electronic form will need to be created to enter data from a tablet pc (or notebook pc) and store the data on the PC. 2. The application should include a means for uploading the data when an internet connection is available. This can be automatic or manual. 3. The uploaded data will then need to be analyzed through the application. The analysis can take place on our server or the data can be downloaded to our office computer and analyzed on our PC. Either approach is fine with us so we will leave it to your discretion but keep in mind that if the data resides on the server it needs to be somewhat protected (the data is not highly sensitive but should remain private). 4. The analysis that will need to be performed on the data is simple and consists of a few basic formulas that we will provide to you. There are a few variables in the formulas that change periodically so there needs to be a way for us to easily change these variables. 5. The user should be able to select whether to (a) view the results of the analysis on-screen, (b) print the results in a report, or (c) download the results to a spreadsheet. The user should also be able to view, print or download the raw data that was collected. We are looking for something simple that will get the job done. You can decide the approach that you think will be best for creating this application so when you submit your bid please indicate in a message the approach that you plan to use and whether the analysis will take place on the server or on our office PC.
