Parse value from URL string/populate page

Hello! This project is for PHP coding. I have a web form that is passing the values of the form submission to the next page. I have a thank you page designed but I need you to write the code to dynamically change the page based on these values submitted. For example, one of the form values is a numerical ID. I have images on my server that match up with these, i.e. [login to view URL] that I want to display as the header on the thank you page. I also have a button on that thank you page for them to continue onto a website and I want that URL to change based on that ID. I will need your help in figuring out how to do that. I have the form and thank you page already designed. Thank you page is in html so you will just need to make this in PHP. I would like this completed within 3 days.
