I'm looking to create a cool-looking form with 3 different types of filters. A sample of what it would look like can be found here:

The form will work as follows: The first section will have the option to choose day, month or year, OR they can use a calendar to select a date range.
The second section will allow the user the option of dragging / dropping certain product types. The default should be that all products are selected, and once an item is dragged and dropped it should be moved entirely.
The final section should have two options: show all, or filter one a single name through a drop down list with an autocomplete field for "name". You can create you own list of say 5 random names to be used as an example.
Once the user hits "submit" the page should refresh itself and display the results of what the user selected on the screen - so the form needs to submit the variables and display them (doesn't matter where they are displayed).
