Barcode Creating and Scanning App and Website

We are looking for someone to help us build an APP that is able to scan a barcode and hit our database to retrieve information. We need someone who is familiar with barcodes as we also need to dynamically generate them. This job will include building out a database driven website that dynamically generates barcodes or "product codes" that will be read by the app that acts as the bar code scanner. Once the bar code is scanned, a signal needs to be sent back to the database so it performs the proper action. The database needs to hold, client info, their product info, and fields for customer responses (name, e-mail address).
