API SOAP Put form to Debitsuccess

Write a HTML user form to take data with the following fields and put this data into the DebitSuccess system via their documented API's:- Company / Business Name * First Name * Last Name * Address * Suburb City * Contact Name * Contact Phone * Contact Email * Invoice / Statement Number/s * Invoice Date * Total GST Incl Invoice Value * ( this field we will calculate out into a set of either 3, 6, 9,12 instalment payments.) The formula to calculate will be:- FurtherPayments = (( GST Incl Invoice Value x Interest Rate ( we provide this field from our site ) x Instalments ( we provide) ) + Gst Inc Invoice Value ) / Instalments FirstPayment = FurtherPayments + SetupFee ( we provide) We will also provide from our site, a custid field that we need to send to DS for each transaction record. Can you set this up as a form for us soon. Hard code the variables that we will offer from our site for the meantime SetupFee = 65 InterestRate = 1.85 Instalments=6
