Avery Labels Printed From Website on Dynamic PDF or MS Word file.(repost)

We have a web based application that stores customer information in a single SQL Server 2008 R2 table. Customer information is essentially name and physical address. The address is broken out in normal address fields. I can give specific field names to the winning bidder. We want a web form that they can execute from the website that grabs all of these names and addresses and creates a template that can then print to Avery 5160 labels. They are doing this currently from an Access database that prints to the MS Word Avery 5160 template (see attached). For our solution we would prefer this to create a dynamic PDF but if creating it to the MS word template is easier that is ok as well. We will edit this code ourselves to add a date range filter at a later point. the important part is to get data from a database formatted and printed correctly on these 5160 labels. ## Platform Windows, Excel 2007
