blackberry iphone send phone number

I need someone to design a mobile application that will work on blackberry and iphones. It simply needs to allow the mobile user to click on a phone number in their phone log and forward that number to a website form. The phone number should be prefilled into the form where the user will continue to enter other information about that phone number via their mobile browser and then click on a submit button and navigate the mobile website from there. I can design the website interface in php using get or post, but know nothing about coding smart mobile devices. Initially i'd like one for blackberry and iphone, but would also like one for any windows mobile device. Thank you. ## Deliverables I will setup a web page with a simple form for testing. The mobile user should be able to download the program from my website then begin using it. I do not require an expiration or trial period for the software.
