Customizing PHP/MySQL autoresponder script (repost)

Needing an update for a autoresponder mySQL/php script you customized for me in the past. I'm needing the ability add unlimited questions and answers using dropdown menus, radio button lists and textbox menu which can all be used as custom variables just like already being done for the text field in script you did before And I would like the ability to pass the variables onto other pages on my website as well as custom fields being sent through autoresponses. Example: Vistor signs up for a free report and I collect their first name, last name, email, mailing address, biggest question about topic and whatever fields I choose will show up on the next page once they hit the submit button personalized from what they typed into the form fields.
