Contact Form Generator

I need a full fledged contact form generator like [login to view URL] where users will be able to specify the number and type of fields and where the messages should be sent. Later they should be able to download the source code. Forms should have Captcha protection. ## Deliverables **Detailed Description of the Project** 1) Once the user opts to create a form, he'll be provided with a basic form (Name, Email, Subject, Message). He should be able to add new fields (with option of selecting the type of fields - Text, Text Area, Check box, Radio, Drop down) or remove existing ones and also specify whether the fields are mandatory or not. He should also be able to change the order of fields. Ajax would be preferred here. 2) When the user is satisfied with order and the fields, he should proceed to the next page where he'll enter the email where the form should be sent and prefix to add to form message (when the message reaches user inbox, this prefix will be appended to the form message subjects). 3) Then in the next page, user should be able to select the style of Captcha (background/fonts) from a set of styles. 4) Finally user should be able to download the entire form files and instructions to install the form on his site. Ideally installation should be such that all files should be uploaded to a single directory and the form should be added to the user website using PHP include function. Kindly have a re-look at the project. The following details have been modified: 1) I had mentioned that I prefer Javascript to Ajax while adding/removing form fields. I now would like to do it with Ajax only. 2) I had mentioned that after selecting form fields, users should be able to select styles for the form. I have cancelled that option. After form field selection, users will go to select Captcha style. Forms will have a generic look.
