create authorised forms for data entry

refer to the document I've attached. Turn this into a data entry webpage, set up to users (at this stage a simple PEAR Auth wrapper is fine) This data will be stored in a mysql database. You can see the information to be entered below, this is a form that people will login to fill out, daily, weekly, or however often they choose. Form entry should be simple, little click up and down button to increase or decrease counts. each week is a new form, you can view past weeks, or summaries, but this is after the basic data entry version is done. Sometimes, people we enter previous days int he week later on etc. This needs to be easily done. (perhaps expand each day if clicked on, show CURRENT day largest. NAME section would obviously come from their login details. (don't worry about an admin section for users yet, but will need a user table of course) week would be calculated, start date, end date etc. database is quite simple, day of year, and data_launches, data_piques, data_repsqualified, data_newpaidibos, and so on.
