Create Custom HTML5 Video Player and Saving Its Statistics in a MySql Database.

Hi,
We would like someone to create the following:
1) Custom HTML5 video player. The visual design prototype will be given by us, which would include a PSD with layers, which will have designs for everything that goes in the player like the play/stop/pause buttons, video seeking slider, volume control button and slider, fullscreen button, etc.
2) Create and populate a mysql database which would save the video statistics such as total frames in the video, number of times video started playing, how many times (at which frame) it was paused, how many times it was seeked, ip address of user requesting the player, etc.
3) Only one video will be used for this project to limit complications.
4) The video player must work in all compatible devices. (For e.g. when mp4 is not listed as first option, it does not play on ipads. These things should be known.)
5) Only one player has to be created as per our design.
POINT NUMBER 2 IS VERY IMPORTANT. DO NOT BID IF YOU CANNOT SAVE THE STATISTICS OF THE VIDEO IN A MYSQL DATABASE.
