Custom Joomla 1.5 Content Module

hi, I need a special joomla (v1.5.7) module to be developed. Is a table with 1 row and 3 columns. 1. column one - drop-down select list of all joomla sections - initially his value is: "Choose Section". 2. column 2 - drop-down select list of the categories of the section choosen at step 1! 3. submit button with label "Go" and sending you to the selected category in blog format. 4. the module must have the parameters available when you creating a table category link at the menu manager - see image.
