Create web form; Install various software and code; Tweak site to display and work properly on all major browsers

I have a web site -- [url removed, login to view] -- which is almost complete. However, I need someone to do a few final things:

1. "Sign Up" Form. I need a web form designed exactly as I have described in the *ATTACHED DOCUMENT*. The form data need to post to an open-source mailing list software program called PHPList ([url removed, login to view]). I also need you to install and configure PHPList on my server. A copy of each form submission will also need to be emailed to my email address. (I believe that PHPList can be configured to automatically send me this email.)

2. "Share This" code. I would like the "Share This" code (available at [url removed, login to view]) installed on this site in the bottom right corner of the page. I will supply you with the sample code from this site.

3. "Contact Us" Form. I need you to create a Contact Us form using the sample code available at [url removed, login to view] ... This form will ask for “First” and “Last Name”, “E-Mail Address”, “Telephone Number”, “Question or Comment”. It will need to have form field validation. It should match the style of the "Sign-Up" form as closely as possible. If the user previously completed the "Sign-Up" form, a session cookie should be set that stores the First and Last Name, E-Mail Address, Telephone Number. That way, if the user clicks the Contact Us form, his information is already pre-populated into the form. This form must be secured from spambots by using the technique described at: [url removed, login to view] ... Sample code is available at that URL.

4. Site Tweaking. I need the site tweaked so that it displays and functions correctly on all web browsers. The site loads correctly for me using Firefox 14 at 1680x1050 resolution, but other browsers and resolutions must be tested. The site will need to be fixed as necessary. The text on the screen should be vertically centered, but right now it is appearing too high on the screen on some browsers. The slide transition is not advancing to the next slide on some browsers when the user clicks on the left or right arrow.

If you do a great job for us, we will have additional projects on this site and others that we can offer you. We are looking for a long-term relationship with a skilled programmer.
