Create PHP login script for website page with editable database, online order form,

[Needs][1] to match domain layout of [[login to view URL]][2]. From all pages, ..Want login, ... to access to my wholesale pricing page ....and order form. Needs to build access page and database that I can edit, add/delete, for entry. After login, they need access pricelist and "order form". Order form needs to be created and designed so it can be printed by them and is sent via email to me. I need to be able to read it easily; maybe in the form of a spreadsheet, not .xml file, I need to be able to give header names on spreadsheet so I can import order to accounting software, Quickbooks. Conversion can happen when I open it. On wholesale page, need to add "Item pop up window" with click on names of distributors, with option to close, print or "email me" the information. Need .css script to allow mouse click at top left of all pages to take back to home home.
