Business advisory on-line tool

I am looking for some software to be developed. The basic elements will be to capture some demographic details of the person undertaking the exercise. Once this is done, the software should move to a screen where there will be up to 100 activities to sleect from. The perosn must click and drag between 10 to 15 of these to a box which represents their answer to a question. If there are to many the software will not proceed until the correct amount are there. It will move to the next question which will be the same process. There will only be 2-3 questions like this. Once the person has completed the questions, the data is then stored and analysed. each activitity will be assigned a level. Once all resposnses are in, (a number of people will be ansswering these questios from within an organisation). The software will identify the 10 most common responses for each question and then plot a dot on the level these activities relate to. for eg if having a drink was the top resposne and it was level 4 then it would place a ot on level 4 of a diagram that will be provided. Each of the top ten responses are plotted and then listed in order of importance below the diagram. the software has to have the capability of allowing me to enter the activities and allocating the level. The software needs to be scaleable and handle up to thousands of replies. It also needs to be able to produce a standardise report including the graphics of the 7 levels and plots.
