I need a Ajax/PHP/MySQL/JSON/XML based questionnaire on Thickbox. See the attached screen. 1. Multi-page Questions are getting displayed from calling a routine in mySQL 2. Score is captured in another table from out of 7 radio button levels 3. Need pagination on the Grid 5. After 'Submit', If the questions left, a red box will get displayed around the question. 6. While moving to the Grid back and forth, the responses of the user should not get lost. 7. Before 'Submit', user can change any responses. 8. Fast loading of the questionnaire grid. Must use Ajax. 9. Need flexibility(functions) to change the number of rows per page, column hide/un-hide, column name change, sorting, filterring etc in the script. I will provide transaction table to store responses, a routine to get the questions for the grid. The output of the work must not less than expected as per screen.
