Development of small PHP / HTML / JQUERY app

Using PHP / Jquery, this is an application to take user input and produce a small report. Essentially it is just 2 pages - input and report. INPUT PAGE There will be 12 questions for the user to answer. Each question will consist of 2 statements. The user will have to allocate 10 points between the 2 statements. For example: I am: (a) calm (b) optimistic. The user would allocate 10 points between a and b. For example, 6 to a, 4 to b. The points will be stored to variables. The allocation of points should be via a slider (Jquery preferred if using a framework). By moving the slider, the total for a and b would adjust. It should begin in the middle with a score of 5 for each. As it it dragged to the left, the numbers would update - 6 for a, 4 for b, 7 for a, 3 for b etc. At the bottom of the page, the user will then click a submit button which will take them to the Report page. REPORT PAGE The 'scores' will be aggregated into 4 groups. For example the scores for Q1a, Q3b, Q8a, Q9a, Q10b, Q12a will be added to produce the total for Group 1. These scores will then be used to produce a page that shows a simple bar chart for each of the 4 scores together with an interpretation of each score. The interpretation text will be based on whether the score is regarded as low, medium or high. Thus there will need to be a simple IF / CASE evaluation for each of the 4 scores to determine which interpretation text should appear. A simple image of how the REPORT page might look is shown in the attached image file. The actual interpretation text would probably be a little larger than this but no more than 4 or 5 sentences. The page must be capable of being printed onto a single sheet of A4 / letter paper. Would also be nice to save to PDF. The pages will should be tablet-friendly. Thanks for reading!
