Create order form + user maintenance page

For a project I am currently looking to create the following: In the theme Minamaze ([login to view URL]) I would like to create a page that allows users to select an Airport, with Google Maps integration, and pick certain fields to then continue to fill in a registration form and send the details. Certain auto-responses need to be sent upon sending the completed registration form. The form needs auto-completion forms. The airports will be entered manually, so I'd need an option to add/edit/delete airports from the list. Full details + mockups will be supplied as soon as we've agreed on the price.
