Contact Form FLASH (PHP) Configure

I am looking to have this project completed ASAP, ie today. I have a flash template with a contact form. The contact form came non-configured. I just need someone to edit the .fla that I will provide. I will send you the .fla, .swf, and [login to view URL] files. You simply need to edit the already made form so it actually sends. You will also need to make a [login to view URL] file that the flash will send the information to. You should test this on your server before telling me it is complete so when I receive it, It works perfectly. I would also want a short readme file telling me how I can easily add another form and what parts of the script I need to update to make it work with the changes.
