Code for pop up screen

I would like the code for a small pop up window to be displayed whenever someone tries to close down the window on my webpage or typeover the address. The popup is to ask a question and give the option to select one or all of the answers, with an option to select an 'other' option and write a few words on what 'other' actually is. I need to be able to change the question and the answers. I do not need to record any identifying information of the person answering the questions. The answers need to be emailed to a specified address (can't type it in rentacoder) with a subject heading: feedback: why I left your site If they click on a link within my site to order (goes to clickbank), the pop up should _not_ come up. I use frontpage to manage my website so would appreciate your guidance on where to input code.
