Automotive info gathering

This is a simple info gathering website. On the index page, the viewer will be asked to select a make and model of car they are interested in buying. On the second page, they will enter a name, email and phone number. This will generate an automatic email to them. All data entered must be stored in a MySQL database. The script language should be PHP. The website should generate an "admin" page that shows the information collected. There also needs to be a way to turn the auto email on and off and change which final page the person is taken to by car maker. For example, if the person wants a "ford" they get the email and are taken to the "thank you page. If they want a "honda" they get no email and are taken to the "sorry" page. I will provide an account with PHP, MySQL, and the domain name so it can be installed by you. I will also do the overall look of the website, logos, etc. If this test is successful, a more complicated website will be needed, so if this is done well it could result in much more work fairly soon. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform HTML, PHP, MySQL- most be complted, tested, and installed on my server.
