Access Report needed

I have an Access database that i will provide. i need a report that totals up each type of different description. For example i need to know how many #42 ALLERGY tsts there were in 1998. this includes adding togeather all the units. I have a sample query in the database. this needs to be seperated by date and year. I need monthly reports and yearly reports from 1988 to 2004 i believe the proc_code can be used as a id for the different type of procedures. ## Deliverables Please deliver in the same database, a seperate modifiable report for each month and year. All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform This needs to run in microsoft Access 2000+ on windows XP.
