DB solution, Questionnaire in 3 days - see info before bid.

Hi! We want a skilled person to set up an online questionnaire/survey where users also can make their own questions toghether with the other default questions. Results must be manageble in several ways along with saving data for each year. This is for school use where we want each class in a school to submit and administer data. Several schools may take part and results of the survey must be taken out global (total for all schools), for each school and for each class. We also want to take out results/sort by other criteria, for instance zip-code, age, sex. First, please see the attachment and tell if you can make any sense of it.
