convert data from spreadsheet to MySQL database

The data is stored in Excel spreadsheet, I want a backend webpage which I can select the excel spreadsheet file from my local computer and import to the mysql on webserver, the requirments are:
1. the data from spreadsheet need to be displayed on the page with a checkbox before insert into mysql, once I tiick all the records I want, then I click a button, the script should only insert the records I selected.
2. some colmuns in spreadsheet needs to be combined in one column in mysql, eg. address line1,address line2, zipcode need to be put together into one column "address" in mysql separate by comma.
3. after all the selected records inserted into mysql, a new spreadsheet file with only I selected records need to be saved in another foler on my computer.
