PLEASE READ FULL DESCRIPTION BEFORE BIDDING You will be required to create an application form that closely resembles this one: [login to view URL] HOWEVER the form that you will create must be written in HTML5 and conform with the responsive SKELETON FRAMEWORK. You will be required to provide all HTML5 and CSS3 markup AS WELL AS THE PHP SCRIPT that generates an excel file output with the captured data. All fields must be verified to ensure that data is in the correct format -- ON PAGE AJAX ALERTS to advise of any input errors. Form must be FULLY SECURED from injections. SKELETON FRAMEWORK and CSS FOR AJAX ALERTS will be provided. You will be required to provide/create any additional markup or javascript as needed.
