Convert Windows Forms to CSS/HTML

We are looking for a talented CSS/HTML designer to do a conversion job that will lead to much more work. The project will consist of converting Windows forms to CSS/HTML using screen shots of the Windows forms. They need to match as closely as possible but we understand the limits of CSS/HTML. We only need the CSS/HTML code, no backend code is required. **You are bidding on the completing the attached form only so that we may evaluate your abilities!** We will use this example to evaluate your skills and to choose a worker for the entire project. Once a worker is selected we will have about 30 of these forms to complete in the next job. And once the 30 are complete there will be over a hundred in the next few months and will be setup as new jobs. We have attached a simple form, most are more complex than this sample.
