Add a bunch of textboxes and html (no graphics) to page from a static .jpg.(repost)(repost)

You need to do this:
1.) Open a .jpg of a screen shot of an old website.
2.) Re-create that page (it's one page) in HTML only. Doesn't have to do anything...just look proper. There is no design really except getting the buttons, links, and textboxes to appear properly.
3.) Do this in VS2010 so I can import the page into VS2010 when I get it from you. No codebehind needed. So what I mean here: Is really nothing. Just "do it" in a VS2010 page. Takes 2 seconds.
Now this page may have 50-70 "controls" on it...like textboxes, inputs, buttons, etc. but that's it.
Must use decent CSS. Screen Must look perfect and identical in Chrome, Firefox, IE, and Safari. It's just straight HTML so shouldn't a big deal.
